Time Warner missing channels "Subscription Required"

I'm in SoCal and installed a CableCARD and Tuning Adapter with my HDHR-Prime. I get most OTA HD channels, but many HD (and even "low res" ) channes don't play, giving a "Subscription Required" message. My Card looks good. Status shows:

Card Authentication
success

Card OOB Lock
success

Card Activation
success

Tuning Resolver
ready


I've had 2 Techs in and have called the support number for them to "send a signal" to the Card. No Joy. I get maybe 1/2 of the channels in my (their lowest) tier.

If you log into the web interface of the HDHR and access the log page, what is displayed there? Have you opened a ticket with SD Support?
